Is a lapse of memory a sign of Alzheimer?s?

I was making a mental packing list for a fall weekend at the beach: sweatshirt, jeans, sunglasses, those sandal things?you know, for walking on rocky shoreline? Kind of ugly, but useful? I could picture them clearly?the rubber soles and webbed uppers?but their name escaped me entirely. A person?s name, maybe? I started to go through the alphabet letter by letter, hoping for a clue. But it was gone. Not ?wait, wait, don?t tell me!? Not ?on the tip of my tongue.? Just gone.
This kind of lapse has always happened to me now and then, especially when I?m very busy, and happens more frequently as I get older. It happens to my patients, too, and they come in concerned that it may be the first sign of Alzheimer?s disease or some other form of dementia. It rarely is.
Half of people over 65 notice more frequent episodes such as I?ve described, where they can?t think of a word or a name. Another common complaint is misplacing items like keys, or entering a room and forgetting why you?re there.
Normally, these lapses?I hate the term ?senior moments? because they happen to non-seniors as well?are isolated, temporary, and not progressive. In other words, that name does come to you, you do figure out where the keys are, and the problem does not spread to other brain functions. Also, the sense that you?re having difficulty remembering things is subjective?your friends and family may not notice.
In early Alzheimer?s-type dementia (the term is qualified because only those who have specific pathological changes in the brain are said to have Alzheimer?s), family and friends may be more aware of a person?s memory deficit than he or she is. It?s not only word (or key) finding that?s affected, but also the ability to do certain familiar tasks such as tying a tie or following a recipe or card game. Recent information, such as what you ate for breakfast, is more likely to be inaccessible than remote data, such as the names of your elementary school teachers.
Here is nice summary of ?normal? memory loss vs. signs of dementia.
Some research suggests that older people who have subjective memory loss of any kind are more likely to go on to have dementia in later years. But given the very high percentage of older people who complain of forgetfulness, it?s hard to know who should be especially concerned. And since treatments for dementia once it occurs are not very effective, prevention rather than screening seems more valuable, at least for now.
Here are some steps to help prevent dementia and they include exercise, reducing saturated fat in the diet, adequate sleep, and other measures recommended for general health.
For those seriously concerned about memory loss, a physical exam and blood work may help diagnose conditions such as thyroid disease, vitamin B12 deficiency, and vascular disorders that might be contributing to the problem.
For those, like me, who have occasional forgetfulness, try slowing down, stop multi-tasking, relax, and give it a few minutes ?

Justice Dept.: OK with new review of health law

(AP) ? The Obama administration told the Supreme Court on Wednesday that it does not object to reopening a Christian college's challenge to President Barack Obama's healthcare overhaul.

In court papers, the Justice Department said it does not oppose allowing a federal appeals court in Richmond, Va., to consider the claim by Liberty University in Lynchburg, Va., that Obama's health care law violates the school's religious freedoms.

A federal district judge rejected Liberty's claims, and the 4th U.S. Circuit Court of Appeals ruled that the lawsuit was premature and never dealt with the substance of the school's arguments. The Supreme Court upheld the health care law in June in a ruling that also said the appeals court was wrong not to decide the issues.

The justices used lawsuits filed by 26 states and the National Federation of Independent Business to uphold the health care law by a 5-4 vote, then rejected all other pending appeals, including Liberty's.

The school made a new filing with the court over the summer to argue that its claims should be fully evaluated in light of the high court decision. If the justices agree, they probably would formally throw out the 4th Circuit's ruling and order it to take a new look at Liberty's case. Liberty is challenging both the requirement that most individuals obtain health insurance or pay a penalty, and a separate provision requiring many employers to offer health insurance to their workers.

The appeals court would have the option of asking the government and the college for new legal briefs to assess the effect of the Supreme Court ruling on Liberty's claims before rendering a decision.

iPad Mini reviewers agree: It's a mini iPad

Apple's iPad Mini will be available for purchase this Friday, and the first batch of reviews has hit the Internet. They seem to agree on a few points: It's built?like an iPad, the screen is a step down, and it's the most hold-able tablet yet.

Walt Mossberg of the Wall Street Journal says, "You can think of it either as a low-cost, compact cousin of the full-sized iPad, or a costlier, larger-screen alternative to the 7-inch models." And indeed, whether the iPad Mini is a luxury or a?bargain depends on what you're considering as alternatives.

The competition at the $200 level is formidable: Google's Nexus 7, Amazon's Kindle Fire, and Barnes & Noble's Nook HD. All offer higher-resolution screens and similarly small form factors. Yet the iPad mini costs $129 more to start. What does it have that the others don't?

According to pretty much every reviewer: The look and feel. "If the iPhone 5 is reminiscent of jewelry, the iPad mini is like a solidly made watch," says The Verge. "When you hold it, it almost feels like you?re just holding a sheet of glass," says TechCrunch. "If there were an industry award for Tablet You Can Most Easily Envision Holding For Extended Periods of Time, the Mini would be a runaway winner," says Time's TechLand. And Fox News gives perhaps the best compliment of all: "When I picked it up, I was reminded of the first time I held a first-generation iPhone."

Clearly, the fit and finish?of the device is superior ? which should probably be expected from something that costs half again as much as the devices it's being compared to.

And then there's the fact that it runs a ton of iPad apps out of the box ? a big selling point for people who don't want to worry about such things when migrating to a new device. "Nearly every larger iPad app I've thrown at it feels usable and comfortable at this smaller screen size," CNET assures us. And the famous iPad battery life is intact, too: Engadget's tests put it at 12 hours and 43 minutes, the best out of all the tablets they've measured.

One thing every reviewer acknowledges but for which?no one seemed inclined?to ding Apple is the screen. The 1024x768 resolution is the same as the original iPad's, yet not only do the iPad Mini's cheaper competitors now?have higher-res?screens, but many smartphones do as well! Reviewers seem to let Apple off the hook by saying that since the screen is smaller, those pixels are packed more closely together than on the iPad 2 or original model.

"Nobody's going to complain about the sharpness," suggests David Pogue at?the New?York Times, but that's probably?not true. Millions have grown accustomed to the sharp displays since the iPhone 4 debuted in 2010, and the iPad Mini's big brother was a huge hit specifically because of the "sharpness" of the Retina display. And while reviewers rightly?praise the Mini's larger 7.9-in. screen in comparison to its 7-in. competition, what they're saying about screen real estate isn't strictly true.

Apple's tablet may have more square inches, but the 1440x900 pixel display of the Nook HD, though smaller physically, can display more words, more clearly ? and the same is true to a lesser extent of other 7" tablets.?

Giving Apple a pass on the screen is a bit questionable, but in fairness to the iPad Mini, its screen is technically more sharp than the?iPad 2's, and by all accounts, this miniaturized iPad is excellent in everything but resolution.

Should you buy it? Screen gripes aside, the sturdy, pretty iPad mini definitely allows you do do a few new things: read one-handed, for instance. Being small and light also makes it a good choice for kids.?

To?really know whether it's worth it, though, it might be a good idea to see the iPad Mini with your own eyes. Of course, once you pick one up, as the reviewers note, you may find it hard to put down ??regardless of your reservations.

If you choose to fight the urge, though, no one will blame you. Given Apple's typical pace in improving its devices,?it seems likely that we'll see a Retina-display Mini by this time next year, if not sooner ? and maybe at a more competitive price.

Internationalised Domain Names EURid UNESCO

The success of the .?? landrush highlights the impact of user education and marketing on public demand.

The .eu registry EURid and the United Nations Educational, Scientific and Cultural Organisation (UNESCO) will present on 6 November a report on the global uptake of Internationalised Domain Names (IDNs).

The study is planned to go out at the 7th Internet Governance Forum (IGF) meeting in Baku, Azerbaijan and it will present updates to the report published in 2011 on the global use of Internationalised domain names which support non-Latin scripts and multilingualism online.

IDNs* have become strongly linked with Internet governance discussions on multilingualism. Of approximately 6 000 languages in the world, only 12 languages accounted for 98% of Internet web pages in 2008, while, according to UNESCO data, English was the dominant language online with 72% of web pages.

According to last year?s joint report, the introduction of IDNs in the Internet root domain was an important milestone in the development of multilingualism on the Internet, especially for non-Latin scripts, including Arabic and Cyrillic.

In addition, the study concluded that IDNs were only one factor in achieving a multilingual Internet, but there was what more to be done to achieve this aim, including securing basic Internet access for the majority of the world?s population, as well as strengthening the environment for creating local language content.

Furthermore, the links between IDN registrations and local languages were classified as ?strong?, as for example the intensity of .eu IDN registrations in Greece (Greek script) and Bulgaria (Cyrillic script).

Last but not least, it was suggested that implementation of key services like email would further benefit uptake, making IDNs fully useable.

On the other hand, keeping in mind that by 2016 around 50% of the world population is expected to be also internet users, the threats associated with growth of domain names increase. Among the most common threats associated with growth of domain names include phishing, counterfeiting and digital piracy.

Interest towards domain names is definitely growing with every single year. The Internet Corporation for Assigned Names and Numbers (Icann) for example published in June this year a list containing more than 1 900 new domain name suggestions. Out of the 1 900 new suggestions, only 166 were of non-Latin scripts.

*Internationalised Domain Names are domain names that contain non-ASCII characters. These characters include, for example, the Swedish ?, the German ?, the Romanian ? and characters from the Bulgarian and Greek alphabets as a whole.

Facebook Mobile Ads: What's Working So Far

In the months and weeks leading up to Facebook?s IPO, investors and analysts questioned the strength of Facebook?s ad-based business model ? in particular, how the company would continue to grow ad revenues as its users spent more time accessing the social network via their mobile devices, where there was less space to run the kind of display advertising upon which Facebook built its business.

Facebook?s third-quarter earnings performance, released last Wednesday, is easing many of those concerns. The social network?s ad revenues grew 36% year-over-year to $1.08 billion, of which 14% ? $139 million ? came from ads running on mobile devices. The company is now making $3 million per day on mobile advertising.

That?s all the more impressive given that mobile ad units in general still command significantly lower price points on average than print and web advertisements. Facebook also only began running its first mobile ads about six months ago. It wasn?t until June that Facebook advertisers were able to purchase ads specifically on mobile.

Mobile ad spending is a small but quickly growing segment of the $500 billion advertising industry. Advertisers are expected to spend $6.4 billion globally on mobile ads this year, a 62% increase from last year, according to estimates from eMarketer. The U.S. market is growing every more rapidly: eMarketer expects a 96.6% increase in spending this year to $2.3 billion.

Types of Facebook Mobile Ads

Facebook has introduced several kinds of advertisements for mobile to date, all of which are integrated in the News Feed. The first of these, Sponsored Stories, recommends brand pages and content based on friends? activities: Users might have seen, for example, a recommendation to Like a brand page their friends Like. Promoted Posts operate similarly, highlighting content from pages that a users? friends Like in the News Feed.

Mobile App-Install ads, introduced in June, are also similar in appearance, recommending users engage with games and apps rather than pages. Users who click on those ads are prompted to install apps on their devices through iTunes or the Google Play store. The most recent new mobile ad unit, Offers, allows brands to attract users through discounts and promotions in the News Feed.

The beauty of placing ads in-line with the content is that Facebook can deliver the same ad experience on both desktop and mobile devices, and unlike traditional mobile display advertising, Facebook?s mobile ads aren?t hampered significantly by the lack of space. They are also delivering higher ROI for advertisers, according to several we spoke to.

Early Success

Marc Grabowski, COO of Nanigans, a firm that buys Facebook ads on behalf of clients including Nordstrom?s Hautelook and Zynga, says the company is seeing click-through rates on Sponsored Stories are 12 times higher on mobile compared to desktop on average, and are 45% less expensive: Cost-per-click (CPC) ads are typically priced between 18 cents and 60 cents, he said.

Why are mobile users clicking more? Grabowski believes it?s because mobile ads have less to compete with. There are no birthday reminders nor display ads running on the right rail in mobile; rather, ads appear only where users? attentions are most fixed: the News Feed. The reason they?re performing better than mobile ads generally is because they are better targeted: Facebook, which has more than 600 million mobile users, has achieved a scale on mobile unsurpassed by any other ad network. Its ads also carry the weight of endorsement; users are only shown Sponsored Stories from brands their friends have already Liked or have interacted in other ways charted by Facebook?s Open Graph, including ?Listened? and ?Read.?

Click-throughs are one metric of success ? but what are users doing after they click? In the case of retail advertisers, they?re buying. Grabowski says his e-commerce clients have invested primarily in Mobile App-Install ads to promote their apps. That?s because users who download a retailer?s app tend to spend more and spend more often than desktop visitors: Clients have told Grabowski that people who make their first purchases on mobile are 2.5x more likely to make a second purchase, and spend 8x as much money with a retailer over time.

Design-focused ecommerce site Fab, which is a client of Nanigans?, has likewise seen strong ? albeit more moderate ? engagement figures. The online retailer plans to spend $25 million on Facebook advertising this year, about 20% of which is going to mobile at present. Fab CEO Jason Goldberg (pictured above) says Facebook generates the best ROI of any other kind of advertising the company does, better than Google, Yahoo and any other ad network it?s tried.

Mobile has proved particularly effective. During the beta test period for Mobile App-Install ads, Fab recorded a 5x better return than any other mobile app download ad run on other networks. Users who register for Fab through one of its mobile apps have twice the lifetime value of web-only customers, Goldberg says. Ten percent of those who join Fab from its iPad app make a purchase in the first week, compared to 3% on the web. The more app installs they get, the more money Fab will make in the long run.

?We?re literally trying to spend as much as possible on mobile at this point,? Goldberg says. Fab plans to increase its ad spending on Facebook by about 30% next year, and expects to shift as much to mobile as possible. ?It?s been highly effective for brands that understand the value of long-term customers, not just transactions,? he adds.

Goldberg says Fab cycles through three or four different ads every day, adjusting the images and copy to generate the highest returns.

Gaming clients are also using Facebook mobile ads to encourage installations of their apps. According to Nanigans ? which, as we mentioned previously, runs advertising for online gaming giant Zynga ? game companies are paying 10% less per install using Facebook mobile ads than they targeted.

Challenges

By all appearances, Facebook and advertisers are striking it rich with the social network?s mobile advertising products, but there?s plenty to be cautious about.

High click-through rates could be attributed in part to the novelty of the ad unit (i.e., What is this?) as well as to accidental clicks, which are thought to contribute to higher CTRs on mobile ads overall.

Scaling is another issue, and one that will ultimately limit how much money Facebook can make from mobile advertising. Ad inventory will increase organically as more of Facebook?s 1 billion users ? 604 million of which are already active on mobile ? continue to spend more time accessing the social network through their devices.

However, Facebook will only be able to serve so many ads without overwhelming its users. Too many ads, and users will become annoyed and resistant.

If more advertisers buy into the platform and inventory doesn?t increase, ads will also become more expensive, and those lucrative ROI figures Nanigans and Fab have reported will crumble.

As such, the success of Facebook?s mobile ad products has yet to be determined.

Meanwhile, Facebook is hedging its bets by investing in other desktop ad formats.
