Different Degrees of Incest and/or Sexual Abuse | Psychology Today

In this post I have followed my usual practice of disguising the patients.

Incest is defined by Webster?s as sexual intercourse between persons so closely related that they are forbidden by law to marry. But religious and secular law has varied from time to time and place to place. What is forbidden at one time may not be at another.

For instance, in some places marriage between an uncle and niece is forbidden. At other times and places it is not. Marriage even between brother and sister has at times been condoned, even encouraged?in Egyptian royalty, for instance, around the time of Caesar.

However, the term incest has come to be used much more loosely today. Different kinds of sexual encounters among family members have merged in many people?s minds with sexual abuse in general. Not surprisingly. Child sexual abuse often takes place between family members.

There is a consensus that sexual experience below a certain age is inappropriate and likely to be harmful. Certainly harmful, most people would probably say. There is not universal agreement, however, of just what age is too young. Children in some cultures can technically marry even before puberty, although the marriage may not be consummated for some time. In this country the age of consent is different from state to state.

As a matter of fact, every sort of sexual interaction between adults and young people, even children, occurs regularly, including rape. The effect of these behaviors is strikingly different depending on how coercive they are and on the relationship of the people involved. Adults who run afoul of the law because of committing one of these criminal acts should not be treated all the same, because their acts are not all the same.

At one extreme: A young woman who was a patient of mine had serious sexual difficulties. Whenever she was at the point of reaching a climax, a picture of her father would flash to her mind. For this reason she was to a considerable extent sexually incapacitated. When she had been a teen-ager, and for some years before, her father had raped her at intervals, not always out of sight of her mother. When I asked what reaction her mother had had, she told me her mother would wave her hands impatiently and tell them ?to stop all that fooling around.?

It was hard to know just what aspect of these experiences was most traumatic: her father?s viciousness, her mother?s indifference, or the unmitigated physical violence, itself. The emotional consequences affected every aspect of her self-esteem and social competence, and plainly was an important factor in her chronic anxiety and depression. She trusted no one.

At the other extreme are many cases of cousins, or even siblings, playing some version of ?doctor,? that is mutual looking, and, perhaps, touching of genitals. Usually, neither child is injured, or even discomfited, by this experience.

I only find out about these incidents years later when they are reported to me in passing. If there was an emotional consequence to them, it was not apparent to me?except, perhaps, for someone expressing a vague sense of embarrassment and guilt.

Those guilt feelings seem to have grown up when these young people grew up and discovered that the sexual behaviors in which they had engaged are condemned by everyone?and if not condemned, exactly, at least discouraged by everyone.

When I started practicing psychiatry, I had not thought very much about these matters. I knew that Freud had regarded sexual feelings as universal among children and central to the development of different neurotic problems; but I did not see very much overt expression of such feelings, so I was inclined to think that Freud?s emphasis on childhood sexuality might have been an odd expression of the Edwardian times in which he lived, or, perhaps, some idiosyncrasy of Freud?s point of view. Incest, itself, was completely outside of my training and experience. I knew there were good genetic reasons for the incest taboo, and I knew that for those reasons that taboo was manifest in many different animal species.

I was, consequently, unprepared when I ran into my first case of actual incest.

One of my first patients was a young, grossly overweight woman who had a variety of social difficulties. She tended to be impulsive and provocative. She fought with her friends and with her previous psychiatrists. She showed up at their houses at odd times asking to use their bathroom. As a consequence, she was told they did not wish to continue her treatment. She had made a number of unconvincing suicide attempts, including one in my office.

Besides these self-defeating behaviors which could be considered aspects of a personality disorder, she had some checking behaviors and other symptoms of a mild OCD. Like most patients?in fact, like most people?various diagnoses could be made to describe her; but they tended to overlap and did not particularly call for one sort of treatment over another. I did not think she needed to be on medication.

One day she mentioned to me that she had not had any sexual encounters recently except with her brother, who was a twenty-four year old divinity student. It took some animated conversation back and forth for me to learn that she had been having sexual relations with her brother for a number of years. It took a number of subsequent conversations for me to come to believe what she was telling me. I pointed out what I thought was obvious, that continuing this incestuous relationship was a terrible idea; and she ought to stop.

?Why?? she asked me.

?What do you mean, ?why?? I think you know why. Incest is condemned by everyone. And you?re both adults!?

?Well, but why exactly should we stop??

Like most things that seem obvious, I had never stopped to ask myself exactly why incest between adults was wrong. ?You know,? I said, finally, ?if you and your brother were to have a child, there would be a high probability that child would be abnormal.?

She laughed. ?Of course, we make sure I can?t get pregnant.?

I thought about all of this after our session was over. The real reason she should stop, I realized only after thinking about it for a while, was tied up with the reason she was engaging in this behavior in the first place. She resented her brother for being a ?goody-two-shoes? and was determined to prove he was just as morally defective as she was. She wanted to drag him down to her level. The reason she ought to stop was to salvage her relationship with her brother?and to stop her systematic attempt to injure him.

It was not a small matter, also, that anyone finding out about her being involved in incest would regard her as unnatural. She knew all this, of course. Sometime later, she did agree to stop. She chose to do this by having me talk to her parents?who had an interesting response, when I spoke to them finally.

?Thank goodness,? her mother said. ?I thought you were going to tell us she was on drugs.?

But, at that point, the incestuous relationship did stop.

I have not sought to study these matters systematically; but as I ran into these sexual behaviors?or misbehaviors?over the years, certain facts have come to my attention:

1. Memories that people report about sexual experiences (abuse) during childhood are not reliable. Certain clinicians have built a world view about early sexual encounters in which they feel that these experiences are the cause of every sort of emotional problem in adulthood. Therefore, they go to great lengths to elicit these stories. Under the influence of a committed therapist, patients will, indeed, remember such experiences even when those experiences did not, and sometimes could not, have happened. I will not go into length here about the ?false memory syndrome,? but these memories are believed in sincerely by patients?sometimes causing the dissolution of families since various family members are accused falsely by the patient of child abuse.

2. However, sexual encounters are common in childhood. In fact, sexual abuse and frank incest are more common than recognized and can occur at any age?from infancy to old age.

When I was a senior staff member at a psychiatric hospital, it came to my attention that the daughter of a patient was discovered engaging in a sexual act with her father who was in a hospital bed at the time. She was 70 years old. I asked her why did she did this, and she told me, ?I thought that was what he wanted.?? I did not think to ask how old her father was or how long the sexual aspect of that relationship had gone on. I think I was feeling world-weary that day.

PHILADELPHIA (CBS) ? A new way to freeze away cancer and a first in Philadelphia. 3 On Your Side Health Reporter Stephanie Stahl is here with exclusive details on the minimally invasive treatment.

This is a new patient friendly way to treat cancer of the esophagus, that?s the tube like structure that runs from the throat to the stomach.
Sonny and Clara Galati have been married 42 years. They have a life filled with laughter, family and exotic travel.

When he was diagnosed with early stage cancer of the esophagus two months ago, they were devastated.

?It was like everything stopped, and I wondered what would we do without him,? said Clara.

Sonny became one of the first patients in Philadelphia to get a new treatment at Temple University Hospital.

?There?s no cutting or surgery involved. There?s no chemotherapy or radiation that?s involved,? said Dr. Michael Smith, who is using a new form of Cryotherapy. It?s called truFreeze, used to zap cancer cells in the esophagus.

?This is a brand new device that maximizes the efficiency of the spraying to allow us to treat more effectively and in a more patient friendly way with less risks of side effects,? said Dr. Smith, who is a paid consultant for truFreeze. He says the patient is asleep during the outpatient procedure.

The freezing spray comes from a catheter guided down the throat with a camera mounted endoscope. Inside the esophagus the frozen, dead cancer cells turn white.

?This is very much a minimally invasive, patient friendly way to kill these cells,? said Dr. Smith.

?There?s no post pain or anything I felt from the procedure. It was very easy to go through,? said Sonny.

And now from fearing the worst, the Galati?s are back to planning more adventures together.

?I?m at the right place at the right time. I?m blessed I could do this, that this is possible,? said Sonny.

Depending on the cancer, patients usually get a series of cryotherapy treatments.

It?s also used for a condition called Barrett?s esophagus, that?s caused by acid reflux and increases the cancer risk.

HP Passport 1912nm Internet Monitor

For businesses that want to offer their customers a fast, uncomplicated way to access the Internet and cloud services, the HP Passport 1912nm Internet Monitor provides a reasonably priced, albeit limited, solution. This unique 18.5-inch display is equipped with an ARM-based processor and uses a Linux OS to access web apps and play multimedia files. It's not a great performer by any means, and it's short on features, but it's cheap and easy to use.

Design and Features
The 1912nm uses an 18.5-inch TN (Twisted Nematic) panel with CCFL (Cold Cathode Fluorescent Lamp) backlighting as opposed to LED backlighting. As such, the matte black cabinet is thicker (2.6-inches) than what you'll find on most LED monitors such as the 1.3-inch thick HP 2011x . The 1,366-by-768 panel is framed by glossy black bezels measuring 0.6 inches on the top and sides and 1.2 inches on the bottom. The bottom bezel is slightly curved and holds five buttons (Menu, -, +, Mode, Power).

The 7.5-pound cabinet has four VESA wall mounting holes and comes with an oval stand that allows you to tilt the panel but offers no other adjustments. At the rear of the cabinet are an Ethernet jack, an upstream USB 2.0 port, four downstream USB 2.0 ports, and two audio ports (in, out). There's also a VGA input for connecting the 1912nm to a host PC, but digital inputs such as HDMI and DVI are missing. A fifth USB 2.0 port is conveniently mounted on the left side of the cabinet right above a card reader slot that accepts SD, MS, MMC, and xD media. The 2-watt speakers are loud and don't distort when the volume is maxed out but they are a bit too trebly.

The 1912nm is powered by a Texas Instruments dual core OMAP 4430 processor running at 1.0GHZ and is outfitted with 2GB of flash memory and 1GB of DDR2 RAM. It runs an HP customized version of Linux and uses Mozilla Firefox as its browser. Video, audio, and photo viewing apps are built in, as is an Adobe Flash player. This is a bare bones Internet device; you don't get any internal storage, it lacks Wi-Fi networking capabilities, and there are no touch screen capabilities. To get started all you have to do is plug it in, connect the keyboard, mouse, and LAN cables, turn it on and you're ready to go.

The main desktop screen contains five massive icons for the embedded apps including Music, Video, Photo, Web, and Settings. You can access files stored on media cards and USB storage devices (ie: thumb drives and portable hard drives) as well as files stored in the cloud. The settings menu is where you go to select photos for a slideshow, set the date and time, configure network settings, and enable the password option.

The monitor ships with an HP USB keyboard and an HP USB scroll mouse, but you'll have to supply your own upstream USB cable and Ethernet cable. HP covers the 1921nm with a three year parts and labor warranty that includes on-site service.

Performance
The 1912nm did a fairly good job of displaying individual swatches from the DisplayMate Color Scales test but its grayscale performance was abysmal. The five lightest shades of gray on the 64-Step Grayscale test were washed out, while over on the dark end the three darkest shades appeared black. This doesn't present much of an issue while viewing most web pages or text files,but photographs and video will suffer a loss of highlight and shadow detail. On the plus side, the screen uses a matte coating that is non-reflective and keeps glare to a minimum.

Viewing angle performance was less than optimal; there was a significant loss of luminance and lots of color shifting at around 50-degrees from dead center. However, small text from the DisplayMate Scaled Fonts test appeared well formed and crisp.

The OMAP processor performed as expected although there was an occasional 3-4 second lag between the time I clicked on a file and the time it took to open it. Web pages were a tad slow to load as well, but other than that there were no major issues.

The 1912nm averaged 22 watts of power during my testing, which isn't all that bad for a monitor that uses CCFL backlighting but still more than the Lenovo D186 (16 watts), an 18-inch display that also uses CCFL backlighting. Neither can match the energy efficiency of LED backlit displays such as the Lenovo LS2421P Wide , which has much larger 24-inch panel but averaged only 16 watts of power.

The HP Passport 1912nm is a good fit for libraries, hotel lobbies, hospitality centers, and any other business that needs to supply affordable web access to its customers. It's easy to set up and easy to use, and it performs as advertised. That said, HP should consider replacing the under-achieving TN panel with a low-cost e-IPS panel that would provide much better color, grayscale, and viewing angle performance. A more powerful processor and wireless networking would be welcome too.

Former SD Gov. Rounds announces run for US Senate

PIERRE, S.D. (AP) ? Former South Dakota Gov. Mike Rounds said Thursday that he will run for the U.S. Senate in 2014, setting the stage for what will likely be a high profile race between the Republican and the Democratic incumbent, Sen. Tim Johnson.

Rounds, 58, said he has worked well with Johnson in the past and considers him a friend. But he said he and Johnson disagree on some key issues.

"At this stage of the game, it's a matter of the direction of this country," Rounds said.

Johnson, 65, has dealt with several serious health issues in recent years that have slowed his speech and required him to sometimes use a motorized scooter when he needs to get around quickly. He had surgery in 2006 to stop bleeding in his brain caused by arteriovenous malformation, a condition that causes arteries and veins in the brain to grow abnormally large, become tangled and sometimes burst.

But in a statement issued Thursday, the popular legislator dispelled speculation that he might not seek a fourth term.

"I feel great, still have work to do and I fully intend to put together a winning campaign in the weeks and months ahead," Johnson wrote, adding that he'd make a formal campaign announcement next year.

Bob Burns, a retired South Dakota State University political science professor, noted that Johnson won re-election in 2008 with 62 percent of the vote, albeit against a lesser-known opponent than Rounds. He said South Dakota voters seem to be understanding of Johnson's physical challenges.

"We know that his mind continues to be keen. While he may take more time to express himself orally than was true in the past, he nevertheless is able to do that," Burns said.

At a news conference announcing his candidacy, Rounds said he is running because he wants to end gridlock in Washington and help reduce the federal deficit, set good farm policies and protect industry from undue regulation. He also said he wants to get rid of major parts of President Barack Obama's health care law that expand Medicaid and subsidize insurance for some people because he doesn't think the government can pay for it.

Rounds said he learned during his two terms as governor that an elected official can stick to principles, such as controlling government spending, and still get things done.

"We need to become a country of cooperation instead of confrontation," he told a crowd of about 100 who showed up for his announcement.

Rounds was a state senator from 1991 to 2001 before serving two terms as governor from 2003 to 2011. After leaving office in early 2011, Rounds returned to his job as president and CEO of an insurance and real estate company based in Pierre.

Johnson was elected to the U.S. House in 1986 and to the Senate in 1996. He was re-elected in 2002 and 2008. He also previously served in the state Legislature.

The state Democratic Party immediately mounted a defense after the morning announcement. Party Chairman Ben Nesselhuf called Rounds a "nice guy with the wrong priorities" for South Dakota.

Johnson was more complimentary.

"I had an excellent working relationship with (Rounds) during his eight years as Governor, and the fact that he has already re-stated his refusal to take the Grover Norquist 'no tax increases under any circumstances' pledge is a very good sign," Johnson said in a statement.

But he added that he still feels great and has work to do.

"In the meantime, I intend to continue to focus on my important work representing South Dakota," he said.

Burns, the political science professor, said Rounds will be a formidable candidate and Johnson represents the Democrats' best chance of keeping the seat.

Rounds became known as "Mr. Nice Guy," after he won the 2002 Republican gubernatorial primary against two other candidates who engaged in a nasty battle of attacks, and Rounds was able to maintain that image through his years as governor, Burns said.

"He left the office of governor without receiving a lot of sharp criticism, but at the same time not garnering undue enthusiasm either," Burns said.

Rounds said he doesn't expect the campaign to begin in earnest until early 2014, but he had to declare his candidacy now to begin building the campaign organization and raising the money needed to challenge an incumbent.

His campaign said his accomplishments as governor included beginning construction of an underground science laboratory at the former Homestake gold mine in the Black Hills, boosting tourism, increasing research at state universities, creating college scholarships, working with the congressional delegation to save Ellsworth Air Force Base from closure in 2005, and helping to make South Dakota a business-friendly state with low taxes.

Rounds said he meant it when he said several years ago he did not want to run for the Senate, but he is running now to make sure his five grandchildren and other young Americans are not saddled with a huge federal debt and have a chance to get an education and good jobs.

"If you can make a difference, you should step forward. That's the reason I'm running. I think I can make a difference," Rounds said.
