Neuro-Oncology: Molecular imaging of pediatric brain tumors ...

Abstract??

Magnetic resonance spectroscopic imaging (MRSI) and 18F-fluorodeoxyglucose positron emission tomography (FDG-PET) are non-invasive imaging techniques routinely used to evaluate tumor malignancy in adults with brain tumors. We compared the metabolic activity of pediatric brain tumors using FDG-PET and MRSI. Children (n?=?37) diagnosed with a primary brain tumor underwent FDG-PET and MRSI within two weeks of each other. Tumor metabolism was classified as inactive, active or highly active using the maximum choline:N-acetyl-asparate (Cho:NAA) on MRSI and the highest tumor uptake on FDG-PET. A voxel-wise comparison was used to evaluate the area with the greatest abnormal metabolism. Agreement between methods was assessed using the percent agreement and the kappa statistic (?). Pediatric brain tumors were metabolically heterogeneous on FDG-PET and MRSI studies. Active tumor metabolism was observed more frequently using MRSI compared to FDG-PET, and agreement in tumor classification was weak (??=?0.16, p?=?0.12), with 42?% agreement (95?% CI?=?25?61?%). Voxel-wise comparison for identifying the area of greatest metabolic activity showed overlap in the majority (62?%) of studies, though exact agreement between techniques was low (29.4?%, 95?% CI?=?15.1?47.5?%). These results indicate that FDG-PET and MRSI detect similar but not always identical regions of tumor activity, and there is little agreement in the degree of tumor metabolic activity between the two techniques.

ahipara girl: Precious things

I have news. We are moving. Its been a very big week to say the least. We've been a part of this community and lived in this home for the better part of eight years. I have for the longest time dreamed of a larger home, with room for the boys and their friends, a craft space for me and bigger living areas for entertaining purposes.?

We have a lot of stuff. Its all my fault. My boys own things like guitars, computers, playstations and scooters. I, on the other hand, am a collector. A gatherer of things vintage. My favourite thing is looking for treasures. So as I scanned my home and inwardly groaned at the idea of emptying the house, plus a very full large garage of all that vintage stuff, plus all my art and craft supplies gleaned over the past SIXTEEN years, to say I was overwhelmed is an understatement.?

However, once we decided where we were going, it became simpler. Sort of. So I have decided to let go of lots of things but keep the absolute treasures I cannot live without. The glass statue my mother gave me, my sister-in-laws hiapo painting, Dagmar's print, my art deco teapot, my favourite coffee pot, some of my silver spoons, my retro coffee mugs and the Italian cut glass mirror also from my mum. My art books and some of my sewing supplies. The antique Japanese sewing box my husband gave me one year for my birthday. A couple of my stitch works and my crochet granny blankets. A hatbox and the glass sugar bowl that winds up and plays music also from my mum. A special quilt my mum gave my son for his first birthday. My Agee jars and a crate of milk bottles. The cushion cover and bunting that my friend Betty-Ann gave me. Some of my vintage prints. My blue typewriter. My camera. My knitting needles and yarn.?

We are off on a new adventure. The destination is a secret a while longer as we hold it in prayer but its going to be awesome and we are more than ready. I am prayerful that this will bless our family. Our time in this place is up and we are grateful for the many memories we have had here. For the lessons learned, the consolidation of our marriage, our unity, our learning as a family. For the friends we have made and community that rose up around us and loved us.?

Titahi Bay has been an awesome place to live. I highly recommend it. The people are warm and friendly. Its a great place to raise kids (though I would suggest to look further afield for high school options). It feels like being on holiday with the ocean mere minutes away, golden sand, swimming in summer and great photos to be had at any time.?

The photos below are symbolic of the kind of haul I would often return with from my thrifting adventures. I am looking forward to new adventures, to new 'old' things and my little artistic self is having a little jump for joy as I anticipate setting up a new home and viewing our new location through the lens of my camera and this blog.?

The silver teapots were $5-$10 each and my silver cutlery was bulk priced about $3.

Travel dos and don'ts for diabetics - Sevensisters Post

Nowadays, the lifestyle and the surge in standard of living make travelling an accolade to everyday mundane activities of home and profession. Be it a family tour or a professional one, ?travelling? always sounds enthralling. According to the Indian Tour Operators? Promotion Council, hundreds of thousands of Indian nationals travel abroad every year to explore foreign lands for varied reasons. Not only outbound travel but domestic tourism is also flourishing in India.

But travelling may not always be welcomed by many people owing to a number of causes, health issues being one of the prime reasons. The current scenario holds a grim picture with many lifestyle-related disorders like obesity, diabetes and heart ailments on the upsurge. The statistical data on prevalence of diabetes illustrates that 40.9 million people suffered from diabetes in the year 2007 and the incidence is projected to increase to 69.9 million by 2025.

Happy journeys and pleasant trips may be a far cry especially for people with diabetes because often travelling comes as an emotional and physical burden for them. They remain stressed about the qualms of their meal and blood glucose management and, more often than not, end with fluctuating blood sugar levels because of erratic meal timings and unhealthy binging.

Virtually, it is simple and easy for people with diabetes to stay at bay from such complications of blood glucose fluctuation. A little rational planning and preparation beforehand can help them sustain their focus on their destination rather than on diabetes.

Some important precautionary and preventive measures for people with diabetes prior to travelling are:

* See your healthcare provider prior to travelling to assess the current level of diabetes control and to get guidance on various diabetes management issues.

* If you are on insulin, make sure to store it in a safe place, away from direct sunlight. Insulin loses its potency when subjected to extremes in temperature.

* While travelling or during a trip, if you eat out quite frequently, take extra care in making right food choices.

* Watch your meal timings and avoid long gaps in between meals.

* Avoid skipping meals as it may lead to hypoglycaemia (dangerously low level of blood glucose).

* Keep some snacks handy while travelling to maintain blood glucose levels.

Tips for diabetics

* Patients with dietary restrictions should contact their airline in advance about special meal options.

*The necessary medications and supplies should be packed in carry-on luggage rather than check-
in baggage.

* Consult your doctor regarding time zone changes, jet-lag and drug and insulin dosage.

* Obtain a travel itinerary showing departure and arrival times, durations of flights, and time differences between the points of arrival and departure.

* Remember to keep a doctor?s letter for Customs officials regarding your medical supplies, in case you need medical treatment overseas.

* If travelling abroad, prepare a list of suitable foods available there in
advance to ease your meal plans.

* Different food and sitting for long periods can make your blood glucose level rise. Remember to test your blood glucose level while flying.

* Stand up and walk around as often as possible to increase circulation, keep blood glucose levels under control and prevent clots.

* Healthy snacks which are fibrerich, low-fat and low-calorie, taken in between the main meals, are ideal for people with diabetes, especially while travelling.

* Stick to the basics instead of binging on fat-rich preparations.

* Baked dishes are better than fried preparations as they offer lesser calories and fat.

* Keep a check on portion sizes while eating out.

* Have fibre-rich foods including whole grain preparations and salads.

* Drink plenty of water and avoid having sugary drinks and alcohol.

Snacking in between the main meals is a life-saving elixir for people with diabetes. Not only doesnacking curb the hunger pangs by enhancing satiety, but it also defendthe person from abrupt ebb and flow of blood glucose. Snacks prepared from whole grainlike oats and ragi are rich in fibrehave a low glycemic index (GI) and slack the process of digestion releasing the sugar gradually in the blood stream unlike simple carbohydrates which promptly raise the blood glucose levels.

Oats and ragi, being good sourceof soluble fibre, have been foundto have cholesterol-loweringeffects, thus diminishing the risk ocardiac disorders. It is best to have baked snackwhich have minimal fat rather than feasting on fat and calorie-dense fried preparations.

Baked snacksenriched with grains like oats and ragi, are fibre-rich; low-fat, transfat-free and low on calorie, and are the best pick for people with diabetes while travelling. Keeping a stock of such snacks before departing for a trip is better than regretting later. People with diabetes can bid farewelto their anxieties about blood fluctuation if a little preparation prior to travelling and adequate precautions while travelling are taken.

US: Syrian general defection a 'crack in inner circle'

Although the U.S. does not want to assume a military role, it does want tougher sanctions. NBC's Andrea Mitchell.

By NBC News and news services

The ?significant? defection of a Syrian general could trigger more high-level defections from President Bashar Assad?s regime, Pentagon and diplomatic sources said Friday.

News of the defection spread as a 100-nation meeting in Paris concluded with calls for global sanctions against Syria to help end 16 months of brutal government repression and civil war that activists say have killed 14,000 people.

Pentagon spokesman Capt. John Kirby called the defection of Brig. Gen. Manaf Tlas a "crack in the inner circle" of Assad's administration.

US Secretary of State Hillary Rodham Clinton, center, listens with German Foreign Minister Guido Westerwelle, left, and French Foreign Minister Laurent Fabius during the "Friends of Syria" meeting Friday in Paris.

Assad and Tlas were childhood friends, and Tlas was considered a confidant of the president.

While Tlas would be considered the highest-ranking member of the military closest to Assad to defect, the general had been placed under house arrest in March 2011 for his opposition to the government's crackdown on militants.

Tlas, a Sunni, had also attempted but failed to bring the Assad regime and Sunni elements of the opposition together.

Some members of the opposition have said they believe Tlas fled Syria only to try to save his family's fortune.

Game-changer? General, close friend of Assad, deserts Syria

U.S. officials could not confirm to NBC News reports that Tlas is on his way to Paris, where many of his family reside.

One U.S. official told NBC News that there was no direct contact between the State Department and Tlas.

Still, Secretary of State Hillary Rodham Clinton, speaking after the international meeting in Paris on Friday, hailed Tlas? defection.

?Regime insiders and the military establishment are starting to vote with their feet" by abandoning the four-decade Assad dynasty, she told reporters.

"We think that is a very promising development. It also raises questions for those remaining in Damascus, who are still supporting this regime."

Clinton also called for Russia and China, which?were not represented at the Paris meeting,?to stop standing in the way of sanctions against Syria.

"I will tell you very frankly, I don't believe Russia and China believe they are paying any price at all,? Clinton said. ?Nothing at all for standing up on behalf of the Assad regime. The only way that will change is that if every nation represented here directly and urgently makes it clear that Russia and China will pay a price because they are holding up progress, blockading it. That is no longer tolerable."

Syria-gate? WikiLeaks' latest drop of secret files

Analysts said the Paris meeting accomplished little despite stepped-up rhetoric.

?There is some solace Gen. Tlas has left Syria,??Aram Nerguizian, a Syria expert at Washington, D.C.-based think tank Center for Strategic and International Studies, told msnbc.com. But the Paris meeting left ?no change in dynamics that is any way measurable ... I don?t think you have a big reshuffle of Russia position.?

Deputy Foreign Minister Gennady Gatilov told the Interfax news agency Friday that Clinton's statement went against the strategy for ending the bloodshed in Syria that was agreed to by world powers last Saturday in Geneva.

That agreement, brokered by international envoy Kofi Annan, said a transitional governing body "shall be formed on the basis of mutual consent." An earlier draft was watered down after Russia stonewalled, adamant that Assad should not be forced out.

Smoke rises from Kerkenez, near Idlib, Syria, on Friday.

Syrian rebels dismissed the importance of the Paris meeting, saying?sanctions weren't working.

They called for military intervention.

Are children fighting on Syria's rebel front lines?

NBC's Richard Engel, who just left Syria after spending a week getting a first-hand look inside the country, says "it was radically different" than the last time he visited now that opposition groups have created safe havens where they are openly in control. In those areas, Syrian troops ? who are now concentrated in pockets within big cities -- could not be seen for hundreds of miles.

At the conference, Hassem Hashimi, a member of Syria's opposition National Council, called for a no-fly zone to prevent military forces from "flying over defected soldiers and civilians and bombarding them."

"We're sick of meetings and deadlines. We want action on the ground," said activist Osama Kayal, speaking via Skype from an area near Khan Sheikhoun.

This article includes reporting by NBC News' Jim Miklaszewski and Adrienne Mong,?msnbc.com's Brinley Bruton, Reuters and The Associated Press.

U.S. Secretary of State Hillary Clinton levels harsh criticism toward both China and Russia over their apparent support for a Syrian regime that may be losing control of the country. NBC's Richard Engel reports.

Map locates where a yacht capsized near Oyster Bay, New York.

(AP) ? A sunken yacht on which three children died during an Independence Day outing must be raised from the waters off New York's Long Island before officials can determine what caused the accident.

There were 10 children and 17 adults aboard the Kandi Won when it tipped and sank following a holiday fireworks show, trapping three children in the main cabin.

Boating experts says the 34-foot-long yacht was too full and was bound to capsize.

A line was tied from police boats to the sunken vessel on Friday so it wouldn't drift away. The U.S. Navy and Coast Guard may help raise the boat, which is submerged 60 feet below the surface.

Yacht owner Kevin Treanor is related to some of the victims. His insurance company lawyer says he'd like to know if something mechanical was responsible.
