Taking painkillers increases death risk, second heart attacks in survivors

ScienceDaily (Sep. 7, 2012) ? Heart attack survivors who take common painkillers after a heart attack have a higher long-term risk of dying or having a second heart attack, according to a new study published in Circulation, an American Heart Association journal.

The painkillers, known as non-steroidal anti-inflammatory drugs (NSAIDs), include over-the-counter medicines such as ibuprofen and naproxen and prescription drugs such as Celebrex (celecoxib), which is used to treat conditions including arthritis.

"It is important to get the message out to clinicians taking care of patients with cardiovascular disease that NSAIDs are harmful, even several years after a heart attack," said Anne-Marie Schjerning Olsen, M.D., the study's lead author and a fellow in the cardiology department at Copenhagen University Hospital Gentofte in Denmark.

Schjerning Olsen and her colleagues used national hospital and pharmacy registries in Denmark to identify almost 100,000 people 30 or older who had a first heart attack between 1997 and 2009, and to see if they were prescribed NSAIDs afterwards.

Forty-four percent of the patients filled at least one NSAID prescription. Among those receiving an NSAID, risk of death from any cause was 59 percent higher one year after their heart attack, and 63 percent higher after five years.

Similarly, risk of having another heart attack or dying from coronary artery disease was 30 percent higher one year later and 41 percent higher after five years. The findings considered other illnesses and medication use in the NSAID patients, as well as differences in age, sex, income and year of hospitalization.

"The results support previous findings suggesting that NSAIDs have no apparent safe treatment window among heart attack patients, and show that coronary risk related to using the drugs remains high, regardless of the time that has passed since the heart attack," Schjerning Olsen said.

Normally, patients who have a heart attack face higher risk of death or another heart attack within the first year. But the extra risk is gone within five to 10 years.

Because the new study instead showed a persistently higher risk over at least five years for patients on the drugs, "long-term caution with any use of NSAIDS is advised in all patients after heart attack," Schjerning Olsen said.

A 2007 American Heart Association statement urged doctors to carefully weigh risks versus benefits when considering NSAID use in patients with a history or high risk of cardiovascular disease.

These data showed no difference in risk for men versus women. Because researchers used nationwide data, the findings extend across races, age, income groups and hospitals.

Researchers didn't test the drugs' effects in a controlled clinical trial, so the study didn't definitely show that NSAIDs, rather than other unknown factors, cause additional deaths or heart attacks. But the researchers consider NSAIDs the likely culprit behind the higher risk.

Use of NSAIDS should be limited and their over-the-counter availability should be re-evaluated, Schjerning Olsen said.

"Allowing a drug to be sold without prescription must be perceived by the general public as a strong signal of safety, and may be contrary in this case," she said.

Good Morning DINKS.? We recently published a post that featured budgeting tips from Fiona Lippey, she is a financial guru who is known for smart budgeting and great savings tips. Lippey is also an author who shares her savings tips with readers on how she lives on only $21 per week.

In an effort to determine how realistic a $21 per week budget can be and because I am a person who rarely turns down a challenge I decided to launch and accept the Dinks $21 Challenge. I wanted to see if I (and our readers) could live on a $21 per week budget just like Fiona Lippey.? I am not going to lie; I honestly didn?t think that anyone could happily live on only $21 per week. But to my surprise, I did.? There were definitely things that I lived without all week as I only spent $21 on myself, but I would definitely not say that I was unhappy.

I constantly found myself looking for coupons and going out of my way to find deals on snacks, food, books and movie tickets.

Depending on how you interpreted the challenge, living on $21 per week could be an average of bulk spending throughout the month or it could a micro budget of actually only spending $21 per week on personal items.

How I lived on a $21 per week budget

Last week I spent my $21 on snacks, a movie rental, and two subway tickets.? I would have enjoyed my $21 a little bit more but I wasn?t feeling well so I had to take the subway to work two days last week.

I definitely gave up a lot of personal items last week such as a new blouse and a new shirt that I wanted.? However I didn?t let temptation get the best of me, even though both tops were both on sale I didn?t buy them. In an effort to stay within my $21 budget I brought my lunch to work every single day last week.? I spent my lunch hour outside with friends instead of shopping or waiting in line for my meal at the food court.

I learned that living on a limited $21 per week budget was not as restricting as I thought it would be.? I lived without some items that I wanted, but those items are just excessive and unnecessary.

I am not sure if I will stick to the $21 per week budget because sometimes I spend triple that amount on myself in a week. Besides, what would I do with all of the extra money that I could save? As our friend James mentioned in a previous post I am not working towards achieving a target net worth.? As long as I save for my future and achieve all of my personal and financial goals I am happy.

Spain PM's bailout comments spur market wariness

(AP) ? Investors reacted warily Tuesday to comments by Spain's prime minister that he won't accept certain conditions in return for a European Central Bank proposal to buy Spanish government bonds.

In his first televised interview since being elected last November, Mariano Rajoy warned Monday night that there are "certain red lines" he won't cross if Spain asks the eurozone's bailout funds for help, which would trigger the bond buying under strict conditions.

He didn't name the conditions, but said separately in the interview that he does not want to cut Spanish pension benefits because retired Spaniards with no way to supplement their monthly state payments would be hit harder than the rest of the country's population.

Analysts warned Tuesday that a drop in Spain's borrowing rate since the ECB program was announced last week might reverse if Rajoy resists pressure to accept conditions that may be imposed in return for the bond buying.

"Given that Spanish borrowing costs have fallen substantially since the ECB announcement last week it would appear that Spain feels less pressure to acquiesce than it did this time last week, thus creating further potential for disappointment as politicians contrive to grab defeat from the jaws of victory," said Michael Hewson, senior market analyst at CMC Markets.

The yield on the Spain's benchmark ten-year bonds was up 0.04 percentage point to 5.72 percent in late Tuesday morning trading. Despite the rise, the yield is markedly lower than when ECB announced its plan.

Rates of 7 percent or more are considered unsustainable, and prompted Greece, Ireland and Portugal to ask for bailouts of their public finances. Spain has accepted a bailout of up to ?100 billion ($128 billion) to save its banking system, which was hit hard by a property boom gone bust. The country wants to avoid a full-blown bailout though.

Rajoy meets Finland's prime minister, Jyrki Katainen in Madrid later.

"Spain deserves to have lower interest rates after all they have done," Katainen said at a Madrid business forum, referring to economically painful austerity measures pushed through by Rajoy this year that have raised taxes and cut government spending.

Spain is in a deep recession with unemployment of nearly 25 percent, the highest in the eurozone.

Chinese President Hu Jintao said Beijing would do all it could to strengthen the 21-member Asia-Pacific Economic Cooperation (APEC) and boost prospects of a global recovery by rebalancing its economy,Asia's biggest.

Russian President Vladimir Putin said trade barriers must be smashed down. He is hosting the event on a small island linked to the Pacific port of Vladivostok by a spectacular new bridge, a symbol of Moscow's pivotal turn to Asia away from debt-stricken Europe.

"It's important to build bridges, not walls. We must continue striving for greater integration," Putin told APEC leaders seated at a round table in a room with a view of the $1 billion cable-stayed bridge, the largest of its kind.

"The global economic recovery is faltering. We can overcome the negative trends only by increasing the volume of trade in goods and services and enhancing the flow of capital."

China's Hu told business leaders before the summit the world economy was being hampered by "destabilising factors and uncertainties" and the crisis that hit in 2008-09 was far from over. Beijing would play its role, he said, in strengthening the recovery.

"We will work to maintain the balance between keeping steady and robust growth, adjusting the economic structure and managing inflation expectations," he said.

Hu spelled out plans for China, whose economic growth has slowed as Europe's debt crisis worsened, to pump $157 billion into infrastructure investments in agriculture, energy, railways and roads. Hu, who steps down as China's leader in the autumn after a Communist Party congress, promised continuity and stability for the economy.

Putin, who has just begun a new six-year term as president, said on Friday Russia would be a stable energy supplier and a gateway to Europe for Asian countries, and also pledged to develop his country's transportation network.

Gazprom, Russia's state-controlled gas export monopoly, signed an agreement with Japan to develop plans for a $7 billion liquefied natural gas plant on Russia's Pacific coast, underscoring Moscow's eastward shift.

RUSSIA LOOKS EAST

The relative strength of China's economy, by far the largest in Asia and second in the world to the United States, is key to Russia's decision to look to the Pacific Rim as it seeks to develop its economy andEurope battles economic problems.

APEC, which includes the United States, Japan, South Korea, Indonesia and Canada, groups countries which account for 40 percent of the world's population, 54 percent of its economic output and 44 percent of trade.

APEC members are broadly showing relatively strong growth, but boosting trade and growth is vital for the group as it tries to remove the trade barriers that hinder investment.

"It is absolutely clear that the most important region for economic growth this decade - and probably the next decade - will be the Pacific," said Mexican President Felipe Calderon.

In U.S., 2012 so far is hottest year on record

WASHINGTON (Reuters) - The first eight months of 2012 have been the warmest of any year on record in the contiguous United States, and this has been the third-hottest summer since record-keeping began in 1895, the U.S. National Climate Data Center said on Monday.

Each of the last 15 months has seen above-average temperatures, something that has never happened before in the 117 years of the U.S. record, said Jake Crouch, a climate scientist at the data center.

Winter, spring and summer 2012 have all been among the top-five hottest for their respective seasons, Crouch said by telephone, and that too is unique in the U.S. record. There has never been a warmer September-through-August period than in 2011-2012, he said.

"We're now, in terms of statistics, in unprecedented territory for how long this warm spell has continued in the contiguous U.S.," Crouch said.

He did not specify that human-spurred climate change was the cause of the record heat. However, this kind of warmth is typical of what other climate scientists, including those at the United Nations' Intergovernmental Panel on Climate Change, have suggested would be more likely in a world that is heating up due in part to human activities.

Alyson Kenward of the non-profit research and journalism organization Climate Central said in a statement, "Extreme heat is closely tied to climate change, and this summer's heat wave left a global warming signature in the data, particularly in the ratio of record high to record low temperatures."

Normally, the number of record highs and record lows would balance out, with an average ratio of one to one. This year, 25 states have had high to low temperature ratios of 10 to one or greater; 14 have had a ratio greater than 20 to one; and three have had greater than 40 to one ratios, Climate Central said.

Ohio topped this list with 49 record high temperatures for every record low.

DROUGHT PERVADES U.S. MIDSECTION

Last month was only the 16th warmest August on record, though still hotter than average, according to the climate data center, which is part of the National Oceanic and Atmospheric Administration.

June was also warmer than average, while July broke the all-time heat record, the data center scientists said in a statement.

It was a dry summer: as of August 28, nearly 63 percent of the Lower 48 U.S. states were experiencing drought. Still, precipitation overall was near the long-term average, with the Southwest and Southeast wetter than average and the Northwest and Northern plains drier.

The Midwest has suffered the most extreme heat, according to Climate Central, which crunches the U.S. government's weather and climate numbers.

Wisconsin, Iowa and Missouri ranked first, second and third, respectively, in terms of extreme heat in 2012, Climate Central said, followed by Indiana, Minnesota, North Dakota, Illinois and Ohio. Two states outside the Midwest - Colorado and Arkansas - rounded out the top 10.

Outside the Lower 48, the U.S. National Snow and Ice Data Center has already reported that Arctic sea ice has shrunk to a record small size, and the melting season is not over yet. The Arctic is sometimes characterized as the world's air conditioner.

As of September 5, the ice on the Arctic Ocean was less than 1.54 million square miles (4 million square km), a 45 percent reduction compared to September conditions in the 1980s and 1990s.

Google discontinuing their pre-paid credit card from Google Wallet

Back when Google Wallet first launched, Google also launched their own, pre-paid credit card for use with the mobile application. Then, just recently the service was opened up to allow the addition of other credit cards. Today, we're receiving emails in our inbox from Google themselves with further Wallet related news. As a result of opening up the service to other cards, the Google pre-paid card is going away. So you need to spend that money, and spend it soon.

It isn't just over, just like that, though. There are some key dates provided that run down to the discontinuation of the card. On September 17, additional funds can no longer be added to the card. Then, following behind a month later on October 17, the card will disappear completely from Google Wallet. If you still have remaining funds on there though, don't panic, you will be able to request a refund. 

One further point to note however, is that resetting the application after September 17 will also remove the card. And it can't be re-added. So, probably better to spend your money first.

The Passionate Foodie: Rant: Restaurant Reviews Ignoring Drinks?

What do you expect from a restaurant review? To me, there are four primary aspects of a restaurant review, references to the food, drink, price and service. Writers should properly address those four key elements in their reviews. There are a number of secondary aspects as well that can be considered such as the restaurant's size, decor, ambiance, parking availability, etc. It seems though that one of the primary elements gets little, if any, attention, by a number of reviewers, from bloggers to print media. Why are these reviewers largely ignoring restaurant's drink programs?

Many restaurants invest much time and effort into developing their wine lists and bar programs. They may bring in experts, sommeliers, mixologists and more, to help design those programs. They may be rightfully proud about their accomplishments, and it becomes a significant reason why diners will patronize their establishment. When considering where to dine, I often consider the drinks menu and I know plenty of others who do the very same. When a restaurant has a particularly interesting drinks list, that should be emphasized in a restaurant review. Unfortunately, that doesn't happen as much as it should.

Let me provide one glaring example. I was recently perusing Devra First's review of Yakitori Zai in the Boston Globe?and was dismayed by her meager coverage of the drinks offered at the restaurant. There was a single sentence in her lengthy review about the alcohol program, which I think is a failure to properly address such a key element. She stated that "Wine is available..." yet says nothing about how many wines, what type of wines, the prices of those wines, etc.

She continues with "...a sake?list offers a good mix of styles and flavors but yakitori may go best with beer." In this, she first fails to mention (and may be unaware) that sake is a traditional Japanese pairing with yakitori, so it would probably make a better pairing than beer. But she also says nothing about pricing, the specific types and and brands of sake and beer available, etc. If you look at the restaurant's online menu, you will see they only list 4 beer options and 10 sake options. So beer doesn't seem too important to them with such a very limited selection. I also feel the sake selection needs improvement, especially that it should include some kimoto/yamahai styles which should pair even better with yakitori.

After reading that review, I looked at Devra's prior eight restaurant reviews, which span July and August, and all of those reviews were available for free without having a subscription to the Globe. Though this is only a small sample, I think it is representative and was disturbed that her reviews showed a pattern of providing minimal information, if any, about the wine and alcohol programs of the restaurants she reviews. Why is this the case?

Of those eight reviews, two failed to mention the drinks at all. 25% of her reviews ignoring drinks completely? That seems wrong. Of the others, the drinks coverage was only 1-3 sentences, which I consider inadequate to properly cover this important element. More information seems to provided about the decor than the drinks. Only one of the reviews mentioned alcohol prices, saying that a wine list is "respectfully priced," whatever that actually means. In all those reviews, a single wine was mentioned by name and only one review specifically mentioned any beers by name. Overall, I would consider this inadequate coverage of an important element of these restaurants.

In addition, prior to my recent visit to the Painted Burro in Somerville, I checked online for other reviews and saw Devra's review?from May. That review merely mentions that the restaurant has tequila, but says nothing else about it or the rest of their bar program. That is a major omission as I later learned the restaurant has a huge tequila and mezcal list, with a number of tasting flight options and plenty of cocktails. In addition, it has a large beer list, with many options from Mexico, and a small wine list. Why didn't Devra say anything about this extensive bar program? After visiting the restaurant myself, I understood how important this bar program is to the restaurant so omitting reference to it in a restaurant review is a significant failure.

Devra is certainly not the only offender and I am using her only as an example. Restaurant reviews should cover all of the major elements of a restaurant, and that includes their drinks program. Failure to do so leads to an incomplete and failed review. You do a disservice to a restaurant by ignoring something of which they might have devoted much energy and attention. You do a disservice to your readers who desire that information. How many consumers decide on a restaurant based on its decor? I am confident that number is but a tiny percentage of the number of consumers who decide based on their drinks program.

Restaurant reviewers, let's see more detailed discussions of drinks in your reviews. If you don't know much about wine, beer and spirits, why not gain a basic education so you can better address your readers about all of the major elements of a restaurant.
